I had to contend with random text formatting (including what I think was white text on a white background in one box leading to disappearing text!), boxes to tick that involved cutting and pasting a ticked box symbol, and random bullet point numbering which then changed the numbers of all future questions when you tried entering a comment. This led to a form which already took around 25 minutes to complete, taking much longer and all past the time I should have finished work.
