Fortunately that is no longer an issue for me (for some 16 years give or take) so the arms do as they please. Choosing which bit of eczema touches the bed is more problematic, mind.

I'd like to be able to put one hand in a claw shape, remove the whole arm, then hold it in my other arm and use it as a back scratcher. And imagine how useful it would be to be able to slide your nose off your face when you have a cold, run your nose under a tap and slide it back on again. Instant relief! Itchy or blocked ear? Just unscrew it, use a Q Tip to clean / scratch it, then screw it back on again.
