I've not yet given up on hoping people might eventually get the hang of 'Rae' - like everyone else has said it's in my email address and signature, and it comes up next to the little picture of me in IM chats...
We had a new guy called Tom who is kept getting wrong, so I started calling him Tim. He gets it right now.

Related - it definitely grinds my gears when someone finds out that the name on my passport is different and starts trying to call me that instead, or, and much worse, try calling me their own short form of it.
There's a few reasons I introduced myself as Rae, and a major one is that I cannot stand being called Rach.
(obviously I don't have a problem with other people being called Rach if they like it, but it's emphatically not *my* name).

Good call Rae! I did that on site the other month: just kept calling the contractors random names until they stopped calling me Hannah...

I get called Alison a *lot*. If I have not introduced myself as Alison (which I won't have, because it isn't my name) what gives people the right to assume what my full name is and call me that instead of what I have said I'm called, or my actual full name which is on my staff badge and in my email address. Even my "Hello, my name is" badge says Ali.

BT WiFi - my phone keeps connecting to it, stopping me from using mobile data. This happened when I first had a mobile, then I've had years of not being bothered, and now it's happening all the time.

The only naming errors on my part have been down to my surname (Hughes), which seems to really flummox Indian call-centre staff, and was once mis-transposed into Huges ... not a bid deal except that it was printed on my "speakers badge" at a conference, so I politely asked them to change it, but I kept the original as I thought it was hilarious
