How do you create them or specify them? I have looked but can't see, and a search on threads didn't answer the question either - but I admit I'm mid-afternoon sleepy and I may be missing the bleedin' obvious ...

Go into "Add training" then click on the red writing "Change your fields".

Though I feel kind of bad about killing your thread by answering the question Maybe some other people could make up some fanciful and implausible answers, like having to stand in a field and turn round anti-clockwise three times with a bit of on your head?

Well at the moment nothing - as I used to just use notes, but with Garmin data filling up those I thought I would note whether I was alone or with a running partner, that kind of thing, and as the half marathon training progresses things like water and/or other sustenance? Open to suggestions as to what others find useful or interesting to look back on!

I think they were added as quite a few people requested a freeform field. Maybe more useful when you could only properly record shoes and not other types of kit ( I use it for which bike I use). Suggestions: prevailing wind; mood of OH; perceived effort level; bowel movements; wildlife spotted, people you ran with; something you might want to note but not show in "who's been training", level of depression, calories used...
