There's a bug I've seen happen a few times, but *never* consistently. I thought it'd be good to have lots of eyes on it, so I'm wondering if you can help me figure out how to reproduce the bug consistently.
What happens is that you type the 'at' symbol when replying to a forum thread. It brings up a box where you can type someone's nickname (to 'mention' them in the thread). You start typing the nickname, and various matches flash up. And then, in some mysterious circumstance, the little square-brackety code appears TWICE or sometimes THREE TIMES in the thread.
I've tried to figure out what makes it do that, but so far I can't make it do it consistently, only 'now and then'. There must be some rational set of circumstances, so I'm challenging you to find out what they are

I'm assuming you're tried different browsers/devices/connections ?

It's both happened and not-happened to me on Chrome - I'm assuming it's not browser-dependent; and on a strong connection.

I've had to catch and delete an unexpected second number several times (I don't think I've ever had triplets), with no obvious reason for why the number has duplicated itself on that occasion when normally the system works fine for me. Seen a handful of examples in the wild too. No idea, I don't know anything about what goes on under the dashboard of the computer.

I can't replicate right now.
From memory it happens when I try tag in a user, I get halfway through writing their tag and I presume once it's unique it closes the pop up. When I've tried to do it now, it's working correctly and the username coming up. But occasionally, I've done the above, the pop up closes but no tag appears in the thread. Then when you go back to try again sometimes 2 or 3 come up. Don't know if that helps you in the slightest.

I've had it happen when it autofills because the characters you have typed are unique to that user and you then also select from the box which then goes blank, making you think you haven't selected anything.

They appear multiple times before I reply. I often just go back and start again anyway.
