Osteitis Pubis - anyone had it?

You're right Iain. Mine referred pain to lower abdomen, and also hip flexor on left side. The abs part was such that doctor thought it might have been a hernia, sports hernia aka Gilmore's groin, but tests showed it wasn't. How's it going Fitz? :-) G

Hi, not been in for a few days. I'm persevering with the physio exercises to improve strength/mobility in my adductors and glutes and have also thrown in a bit of abs toning stuff just because I'm feeling fat from not running :-(

Back to the doc soon to discuss next steps, I'm going to pursue the bisphosphonate infusion option as it seems to be a cure-treatment compared to a corticosteroid injection which seems to be a pain-masking treatment. Whether the medical insurance will pay for it is another matter...

Bloody frustrated. Last time I saw the doc he was explaining either corticosteroid injection OR bisphosphonate infusion as alternative approaches. Today I went back and said I'd decided on the infusion, and have also spent time getting the insurance company to support it.

Today he says he recommends the corticosteroid first, then if that isn't effective the infusion is an option.

Either way nothing happens until I've been back again, this time to see a rheumatologist who would carry out either procedure, to get his opinion and agree a treatment plan.

Last year I was injured for quite some time. My injury was stress responses in various areas but the MRI also showed OP. I wasn’t getting any symptoms from it. From April to September I pretty much rested for the other injury and had 3 MRI scans in that time. The OP did not appear to improve on the scans despite the rest. However I wasn’t getting symptoms from it.
I originally saw a sports DR but when I wasn’t getting better (SR meant to take about 6 weeks) he referred me to a rheumatologist with a view to having bisphoshinate treatment (cortisone injection didn’t work).

The rheumamatologist wanted to do a whole stack of tests before agreeing to the B treatment. They all came back normal so he then wanted another MRI scan to see what was going on ( my 3rd). That come back saying the SR had healed but the OP still there. I was still getting symptoms from the SR area which was odd given it was healed. He said to do some running and work hard on pelvic stability as that was prob why I was still having symptoms due to the weakenss. He said to fix the OP I also needed to work on pelvic stability.

He said come back in 3 months. Have another scan and if OP still there then can have the B.

I got the impression they are not keen on using it. My insurance wouldn’t pay but I would have done. I never went to see him again as I had used up my insurance allowance and I had to go to London to see the rheumatologist ( one of the top in the country which is why I went there initially)
So the message is

1. Work vvv hard on pelvis stability
2. Some Dr are not keen on using B and only as a very last resort

Thanks, Hellen. My physio exercises include things like clams & bridges (with resistance) and I've been adding adductor strength / stretch exercises plus some core / abs work just because not running is making me feel fat!

I'm very frustrated by thee OP injury as it doesn't seem to be getting better and the interaction with the doc is going far too slowly for my liking.

Update:
I've been spending a lot of time in the gym: rowing, abs / core work, adductor strength exercises.

Last week I had the bisphosphonate infusion and have had none of the indicated side effects (touch wood).

2km running on the treadmill today, with no symptoms of OP. I'm going to up the distance by 1km per day and see if I can push on. If I can get to 10km on a treadmill it might be time to try running on the road again.

Back to physio soon for a review of rehab progress.

Fingers crossed. I really miss running! 😢

I think I've developed this. Started to get a bit of stiffness in my glute 2/3 years ago but that co-incided with a return to playing cricket after an 8 year break so thought it was because of that. Have been getting progressively worse pelvis pain and lower abdomen since May/June. Left inner thigh (abductor or adductor?) and lower abdomen pain is pretty much constant now, during run and after. Went to Dr's and he was useless, ruled out hernia and suggested ibuprofen and rest. Did that put on weight feel miserable!!! Have the Royal Parks in a month and desperately want to do it even if its super slow. Did you make any progress Fitz, give me some hope!!!

Don't lose hope TM. Does the IBPF help at all? As the discomfort from OP is largely from inflammation (if I've understood it correctly), anti-inflammatories should help. It's not a cure but it should get you through RPHM - lovely race, by the way, I did it twice but keep missing out on the ballot and not willing to stump up £££ for a charity place.

You need a diagnosis, get your GP to refer you to a specialist and some tests, do you have medical insurance to cover at least the initial investigation and follow-up? Also look at physio or DIY exercises to improve pelvis / glute / hip strength & mobility.

I had my bisphosphonate infusion back in November. I'm back running if not racing, able to run an hour or more and hoping to push on for London mara next year, having deferred my place this year. Recovery has plateaued a bit so currently considering a second go at the infusion. These days I mostly avoid running on consecutive days as it's too uncomfortable.

I hope you get some progress, it really is an infuriating injury.

Thanks for the words of encouragement Fitz. Yes I'll take what I need to get me through RPHM and then I'll get a proper diagnosis, you are right!!

Good luck with your VLM for next year, I hope the training goes well.
