I worked with airport information systems for years and it wasn't uncommon to see a flight "TST001" on the public displays. Probably still happens.

The live and test financial services environments I worked on in the 90s were fairly well segregated, but the print/mail room operators did mistakenly issue some test letters to the addresses of real customers.

The customer who received a letter for ‘Mr Test Case 1’ wasn’t too upset. The holder of an investment bond who received a letter addressed to ‘Mr Tax Dodging Bastard’ wasn’t quite so enamoured.
