Thank you all for your replies. Yes it's OA in my knees and I've been with NHS physio which unfortunately was very poor. I work for NHS and I know that there are good and bad but unable to get a second opinion. I will continue trying different forms of pain management and if I keep it steady perhaps the summer months will be easier

NHS physio is notoriously lacking I'm afraid, their only focus is short term pain relief. I'm afraid you need to visit a proper physio who has specialises in running injuries, but as I said earlier - there is no magic cure for OA. I think your best route is to find pain relief that you can tolerate.

I've been lurking. Do you all still run with OA, but take painkillers to make it less painful? Or have you stopped running? And will running accelerate the degeneration?

My experience of the efficacy of NHS physiotherapy is far more positive than you suggest, NE. NSAIDs are out for me, so the gradual strengthening of muscle supporting the knee worked well, albeit slowly. I've kept up the exercises and I'm running. :)

Thanks, WA. It helped to be advised early on by GP & physio staff that OA 'couldn't be cured' and management of discomfort and inflexibility was the best way forward.

I do run and don't take painkillers any more (used to, but now I've stopped I wonder why!) I can't run fast but nothing really hurts that much.

I'm glad your NHS physio experience was positive Steve, however in my experience it was anything but, all they were interested in was cessation of pain and then shoving me out of the door. That's not to have a dig at the NHS, the rheumatolgy department at my local hospital have been absolutely fantastic and hugely sympathetic, they made my condition much easier to bear. I have seen several private physios and the one I go to nowadays - roughly every four weeks - has been instrumental in some life-changing treatment for me, if it wasn't for her I doubt I'd be running at all and most likely still shoving a 20mg injection of horrible, cytotoxic serum into my leg every Sunday night.

Martyn: I've been reflecting on my comments and I should have thought more about the difference in types of arthritis. Of course, how the patient evaluates any treatment is highly subjective, e.g. different degrees of tolerance to pain, and what might be reasonably expected from a series of visits.

For example, I found out that my knee was a long way from needing an artificial replacement; and Velociraptor counselled against hydrocortisone injections.

I've followed your diagnosis and treatment here on Fetch, and the proof of the quality of your treatment rests in your continued assault on steep ups and even steeper downs. :) No fell is safe!

There are some great comments here and it's encouraging to hear. The fact that you are still running without painkillers gives me hope. I can run without pain it just cripples me in the following days despite my best efforts to practice active recovery. But I won't give up :)
